The person, working together with their family and Care Manager, will create a support plan that best meets the needs of the person and family.  This plan can create supports which promote personal independence, are focused on desired skill acquisition, and/or engage in a wide range of community settings. Hours are flexible. Eligibility: The person must be over the age of 18 and approved for OPWDD services.
